As an Afterhours RN Care Manager at Gentiva, you’ll provide triage, clinical decision-making, and urgent home visits for patients during evenings, nights, weekends, and holidays. You’ll assess symptoms, manage pain and medications, coordinate with on-call providers, and update care plans to support safe, timely interventions. Working independently in the field, you’ll educate patients and families, document in the EMR, and collaborate with Gentiva’s interdisciplinary team.
