As an Afterhours RN Care Manager at Gentiva, you’ll provide triage, clinical decision-making, and care coordination for home-based patients during evenings, nights, weekends, and holidays. You will assess patient needs via phone and in home, manage urgent changes in condition, coordinate with on-call providers, and implement care plans that support safety, comfort, and dignity at home. Working within a compassionate, mission-driven team, you’ll educate patients and families, document accurately in the EMR, and collaborate with interdisciplinary staff to ensure timely, high-quality care and a positive patient and family experience.
